Balancing youth work and disability care, you will create a safe, empowering environment to help them reach their goals. Guided by the Case Manager, you will provide person-centred, trauma-informed care, meet daily needs, and foster emotional, social, and cultural wellbeing. You will assist with appointments, family visits, community connections, education, and life skills, promoting independence as they transition to adulthood.Other key duties will include: Support young people with physical, intellectual, or psychosocial impairments in building independence through personalised care plans.Provide personal care, including dressing, bathing, grooming, medication management, and toileting.Empower young people to develop life skills like meal preparation, household chores, and cleaning.Facilitate participation in community activities and manage the logistics involved.Support engagement in education, vocational activities, and skill development.Help young people build social skills and maintain relationships with family, peers, and social networks.Provide emotional support, apply regulation strategies, and manage distress or escalation.Accompany young people to appointments, ensuring their needs and opinions are respected.Organise and support leisure activities, encouraging hobbies and personal interests.Maintain accurate case notes, complete shift handovers, and follow incident reporting protocols. What you can bring to the team Your experience, empathy, and strong interpersonal skills will be invaluable in providing high-quality support for our youth as they transition into adult hood.We also require: Minimum Certificate 4 in a related discipline with preference given to Youth Work and human services qualifications.Minimum 2 years’ experience in Youth Work, Disability Support or relevant field.Previous experience as a Youth Worker within a disability context with demonstrated experience of trauma informed care will be highly regarded.Proficient computer skills and experience using Word, Excel and Outlook. Hold or be willing to obtain a Working with Children Check (Blue Card), NDIS Worker Screening Check (Yellow Card), Child Safety and Personal History Screening Check (LCS2), First Aid, CPR, National Criminal History Check (Fit2Work), and an open Drivers License with comprehensive insurance on your vehicle. To join #TeamPossible! If this is the opportunity for you, click ‘Apply’.
