We are passionate and committed to securing high performing team members who will embrace and drive our purpose and values forward.The Role:The purpose of this role is to provide Social Work clinical services, as a member of the broader Customer Safeguarding Team, to produce outcomes in support of the customers’ participation in their learning, social, home and community environments.  Clinical services are delivered in a manner that ensures the social, emotional, cultural and environmental circumstances of the customer and/or their carer are valued and fosters professional and ethical excellence.  Duties will include:Provide social work clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics, AASW professional practice and Organisational Practice Guidelines to achieve positive outcomes in meeting customer’s needs.Develop an ability to use evidence based practice for clinical decisions and actively maintain professional skill development.Monitor and report on social work practices and outcomes within the Health Care Team in accordance with the framework.Undertake the role of key contact person for designated customers and complete all Service Planning documentation in collaboration with the multidisciplinary team as appropriate.Manage the development, implementation and maintenance of referrals throughout case management to achieve outcomes for individuals.Operate in accordance with the principles of Person Centred/Family Centred practice.Achieve quality customer outcomes in line with the Human Services Quality and NDIS Quality and Safeguarding Frameworks.Lead and role model exceptional donor communication and stewardshipWhat you need to succeed:Bachelor’s Degree or HigherEvidence of membership of the Australian Association of Social Workers (ASWA)Willing to gain knowledge and skills in social work practices required for working with customers with various disabilities (e.g. individual casework; family work; team work; networking; community work; group work and advocacy; counselling and communication).
